SCENT FREE ENVIRONMENT

Due to severe adverse reactions, I ask that you and anyone accompanying you, please refrain from wearing scented products ( perfume, scented lotions, scented deodorants, laundry products, soaps / body washes, hair spray, strongly scented shampoos etc) when you enter our facility. Because I will not be able to continue working if I’m exposed to scents, you will be asked to reschedule your appointment on another day. For this reason, we ask that new patients call for information / to book vs walking in unannounced.

CANCELLATION POLICY:

If you need to cancel your scheduled appointment, we require at least 24 working hours notice so that we can try to fill your spot.  Failure to do so will result in your account being charged a cancellation fee of 100% of the fee of your scheduled appointment, and discharge if it happens 3 times. You will be asked to pay for the missed appointment prior to your next appointment. This will occur if we cannot fill your missed appointment.

REASONS FOR DISCHARGE:

You will be discharged from care for any of the following reasons. ( this is not an exhaustive list):

  1. You have met your goals and have regained your pre-injury status.
  2. You have decided that you do not wish to return.
  3. Despite both of our best efforts, you have not improved or your progress has reached a plateau. In this instance I will suggest another course of action or  perhaps suggest another medical professional.
  4. Non-compliance with attendance or your prescribed treatment / home program.

MASK WEARING POLICY

Since the onset of covid 19, we have upheld the covid precautions. I have an air filter / fan in the clinic that exchanges the air every 35 minutes. We wear N95 masks and we ask that patients wear Masks that are of N95 or KN95 quality or we will provide one.
